MIST 2008 Registration Form

Important
To complete your registration, follow these 5 simple steps in the order listed below. If you need more information about rules and regulations, see www.getmistified.com.

1- Form a team from your school/community and make sure your coach has filled and signed this coach form
2- Make sure your coach fills out this form for the entire team.
3- Please fill out the online form below and have your coach and parents/guardians sign this form
4- Please give your coach your registration and tournament fee of $25 (which covers dinner on both days, venue, and misc. costs). Please write a check made out to the "Harvard Islamic Society."
5- Have your coach collect the forms and payment from steps 1 though 4 for the whole team and send them to the following address:

Your coach should mail the forms and checks by ***March 31, 2008***(EXTENDED DEADLINE). Not doing so will jeopardize your team's participation in MIST. We will email your coach within 2-3 weeks of receiving your team's payment.

If payment is an issue for you or your family, financial concerns will be accommodated. Please let us know your situation by emailing us at mistboston@gmail.com.
