What is MassHealth?

MassHealth is a public health insurance program that offers to pay for health care for certain low- and medium-income people living in Massachusetts, including a program for individuals who are HIV positive. In other states, it is commonly referred to as Medicaid.

Who is eligible for MassHealth?

There are different types of MassHealth coverage to serve different populations, and eligibility varies for each. To see which type is best for you, please click on the following link: MassHealth Eligibility Check

In order to be eligible for one of the coverages offered by MassHealth, you must fall into one of the following categories:
• a family living with children under the age of 19 years
• a pregnant woman with or without children
• a Dept. of Mental Health client who has been out of work for a long time
• a disabled person
• an adult who is working for a qualified employer
• a person who is HIV positive
• a child under the age of 19 years
• an adult caretaker relative living with children under 19 years of age when neither parent is living in the home
• elderly (65 or older)
• a woman with breast or cervical cancer
• a person in need of long-term care
• a young adult under age 21 who was in the care and custody of the Department of Social Services on his/her 18th birthday

Along with meeting one or more of these requirements, you must be a resident of the state of Massachusetts and meet financial eligibility requirements

How do I apply?

The application is called the Medical Benefits Request (MBR) form. There are 3 different ways to obtain the form.

1. Call the MassHealth Enrollment Center at 1-888-665-9993

2. Get the application from the MassHealth website (links to the applications are provided below):
People under age 65: English | Spanish
People age 65 or older or anyone needing long-term care: English | Spanish
People on Medicare applying for MassHealth Buy-in: English | Spanish

3. Obtain from a Local Community Health Center
Cambridge Health Alliance Center

The following documentation is often required when applying for MassHealth:
• Proof of identity
• Proof of citizenship for U.S. citizens and nationals
• Proof of immigration status for noncitizens (copy of green card or other official immigration papers)
• Birth dates for all family members
• Social Security number (Note: This is not needed for MassHealth Limited.)
• Proof of disability determination
• Proof of wages (two recent wage stubs, copy of tax return, or letter from your employer)
• Proof of self-employment, business, or rental income (copy of tax return)
• Proof of unearned income (TAFDC, SSI, child support, pensions, Social Security)
• Bank statements
• Copy of health insurance card for applicants with health insurance
• Proof of medical condition for people applying based on HIV/AIDS status
• Certificate of blindness for people applying based on blindness

Note: If you do not have some of these documents, you may be allowed to use a sworn statement from you or someone who knows you.

If you choose to mail your application, send it to the MassHealth Enrollment Center listed on the application. Be sure to answer all the questions, then sign and date the form, and send it in.

How long does it take to obtain MassHealth?

If you are not disabled, MassHealth must make a decision on your eligibility within 45 days if your application is complete. If you are disabled, this might take longer, but should not be more than 90 days after submission of a complete application.

If MassHealth needs more information or proofs from you, they will usually notify you within 5 days of receiving your application. You will have 30 days to send in your information.
